Tool: Parent observation form *

Below is a sample form that could be given to parents who come in to observe your class. Using something like this will make these parents feel more comfortable and give you useful feedback on what they are observing and thinking.

Welcome to our classroom. I am glad that you were able to come in to observe today. The students enjoy having visitors and I enjoy hearing from people about what they are seeing and hearing about their child’s participation. While you may have other areas you want to focus on during your visit, listed below are some suggested areas for you to think about. Feel free to add additional notes on the back of this sheet. Please leave your notes with me at the close of your visit or send them to school with your child later. Your feedback is important to me!
